Safe Sleep at 18 Months

At 18 months, SIDS risk is essentially gone. Sleep challenges at this age are entirely behavioral. A small pillow is now appropriate, and many families transition to a toddler bed between 18 and 24 months.

โœ… Safe Sleep Checklist for 18 Month Old

๐Ÿ›๏ธ

Crib or Toddler Bed

If baby is climbing out regularly, a toddler bed transition may be appropriate. Otherwise, keep the crib โ€” it's safer.

๐Ÿงธ

Comfort Items Fine

One or two small stuffed animals or a lovey in the crib is appropriate.

๐Ÿ›Œ

Small Pillow OK

At 18 months, a small, firm toddler pillow is appropriate. Avoid adult-size pillows until age 2+.

๐Ÿ›Œ

Light Blanket Fine

A lightweight toddler blanket is now appropriate. Avoid thick comforters or weighted blankets.

๐ŸŒ™

Bedtime Routine is Everything

18 months is peak power struggle time. A calm, consistent bedtime routine is your most effective tool.

๐Ÿšช

Baby Gate for Safety

If transitioning to a toddler bed or floor bed, use a gate to keep toddler safely in their room at night.

๐Ÿ“Œ Key Note for 18 Month Old

18 months brings a significant sleep regression driven by a developmental leap in language and autonomy. Expect pushback at bedtime โ€” it's developmentally normal.

โš ๏ธ Common Mistakes at This Age

  • Using a weighted blanket on a toddler โ€” these are not recommended under age 5
  • Transitioning to a toddler bed without a gate โ€” toddlers will roam at night
  • Giving in to every request at bedtime, which extends the routine indefinitely
  • Letting the toddler bed be in the middle of the room with nothing to prevent falls

๐Ÿšจ Call Your Pediatrician Ifโ€ฆ

  • Fewer than 10 words at 18 months โ€” mention to pediatrician
  • Extreme sleep resistance lasting more than 4 weeks
  • Child wakes every 45 minutes all night long with no ability to self-settle
  • Any sign of sleep apnea: loud snoring, gasping, prolonged pauses

Frequently Asked Questions

When should I transition from crib to toddler bed?

Wait until your child climbs out of the crib consistently, or until they're 3 years old โ€” whichever comes first. Many children stay in a crib until 2.5โ€“3 years, and that's perfectly fine.

Can I use a weighted blanket for my 18-month-old?

No โ€” weighted blankets are not recommended for children under 5, and many pediatricians say to wait until 7. The suffocation risk is real for young children.

My toddler comes out of their room 10 times at night. What do I do?

If still in a crib, a gate or door handle cover works. For toddler beds, a consistent "silent return" approach โ€” walking them back every time without engagement โ€” is the most effective behavioral strategy.
